Rename Copied PDF Form Fields When Their Answers Must Differ
A form can look perfectly clear while behaving ambiguously. Before changing its layout, write down the information each question is supposed to collect. Two people involved in the same job may have different names even when the corresponding boxes look identical.
Use a fictional form in Acrobat desktop with Prepare a form available. Keep the original template and work on a copy. For this exercise, the two questions are Collection contact and Delivery contact.
Separate field identity from position
In All tools > Prepare a form, copied fields retain their names and share entered values. Moving the box or changing its caption does not create an independent answer.
Rename the second field when it represents a separate answer. Use clear distinct names such as CollectionContact and DeliveryContact, while keeping the visible questions readable to the person completing the form.
Enter Asha for collection and Ben for delivery. Leave each field, save the practice copy, reopen it and inspect both answers. Then change only the delivery answer to Cara: collection should still say Asha.
Decide which repetitions are intentional
A repeated reference may intentionally represent one answer; another person's details require a separate decision. Record that requirement in the template review sheet.
If a colleague reports a repeated answer, ask them to identify both question captions and the exact edit they made. That makes the defect reproducible without requesting real customer details.
Accept the revision only when the two-question exercise matches the intended data collection. Keep the test names with the review notes rather than distributing the filled practice form as the blank production template.
